Privacy Policy

Effective date: October 30, 2026

This Privacy Policy explains how DynaWell Peptides Inc ("we", "us", "our"), the operator of Forma Wellness and the DynaWellness network, collects, uses, discloses, and protects your information when you use our website, patient portal, and related services (the "Services"). Health information you share for a consultation is handled with additional protections, described in Section 3 and in our HIPAA Notice.

1. Information we collect

  • Account and contact information — name, email address, phone number, shipping address, date of birth.
  • Order and payment information — items purchased, amounts, and payment card details (processed and stored by our PCI-compliant payment processors; we never store full card numbers).
  • Health information — the answers you provide in intake questionnaires, identity documents, and your messages with your care team. This information is transmitted to the independent providers and clinical network responsible for your care.
  • Technical information — device type, browser, IP address, and usage data collected via cookies and similar technologies.

2. How we use your information

  • To create and manage your account, process orders, and provide customer support.
  • To route your intake and health information to licensed providers and pharmacies so they can deliver care and fulfill prescriptions.
  • To send transactional communications — order confirmations, shipping updates, care-team message notifications, and refill reminders.
  • To send marketing communications where you have opted in (you can unsubscribe at any time).
  • To detect fraud, secure the Services, and comply with legal obligations.

3. How health information is handled

Your questionnaire answers, identity documents, and care-team messages are transmitted directly to the clinical network and the independent providers responsible for your care, where they are stored in clinical systems subject to healthcare privacy laws. Our platform is engineered so that this clinical content passes through to those systems and is not retained in our commerce database — we keep only non-medical routing records (case identifiers, statuses, and timestamps).

Notification emails we send you never contain medical content — they only tell you that something happened (for example, "you have a new message from your care team") and link you to the secure patient portal.

4. How we share information

  • With independent licensed providers and the clinical network, to deliver your care.
  • With licensed U.S. pharmacies, to fulfill and ship prescriptions.
  • With payment processors, to process transactions securely.
  • With service providers (hosting, analytics, email delivery, fraud prevention) bound by confidentiality obligations.
  • When required by law, subpoena, or to protect rights, safety, or the integrity of the Services.

We do not sell your personal information, and we do not share your health information with advertisers or advertising platforms.

5. Cookies and tracking

We use essential cookies to operate the site (session, cart, security) and, with your consent where required, analytics cookies to understand how the site is used. Marketing pixels are never fired on pages where you enter health information. You can control cookies through your browser settings.

6. Data retention

We retain account and order records for as long as your account is active and as required for legal, tax, and accounting purposes. Clinical records are retained by the providers and clinical network in accordance with medical record-keeping laws. You may request deletion of your account data as described in Section 8.

7. Security

We use industry-standard safeguards: encryption in transit (TLS) and at rest, access controls, audit logging, and tokenized payment storage. Patient-portal access requires a one-time code sent to your email. No method of transmission or storage is 100% secure, but we work continuously to protect your information.

8. Your rights and choices

  • Access, correct, or delete the personal information we hold about you.
  • Opt out of marketing communications at any time via the unsubscribe link or by contacting us.
  • Depending on your state of residence (for example, California, Colorado, Connecticut, Texas, Utah, Virginia), you may have additional statutory rights, including the right to know, delete, correct, and opt out of targeted advertising. We honor these requests regardless of your state.
  • To exercise any right, email telemed@formawellness.com. We will verify your identity before acting on the request and respond within the timeframe required by applicable law.

9. Children

The Services are strictly for adults 18 and older. We do not knowingly collect information from anyone under 18; if we learn that we have, we delete it.

10. Changes to this policy

We may update this Privacy Policy from time to time. Material changes will be posted on this page with an updated effective date, and where required we will notify you directly.
